Job summary

The North Carolina Department of Health and Human Services (DHHS) is seeking a Casework Associate I (S) to serve as case manager for EIPD at WorkSource East Training Center in Wayne County. You will develop rehabilitation plans, manage all training phases, and document services for students referred by EIPD counselors.

Qualified candidates will have a bachelor’s degree and experience in case management or counseling for people with disabilities, and possess excellent written and oral

Qualifications

• Bachelor’s degree in a discipline relative to the program served; or Bachelor’s degree plus one year of related experience; or equivalent combination of education and experience.

Responsibilities

• Serve as case manager for Employment and Independence for People with Disabilities (EIPD), students referred to WorkSource East.
• Develop and implement the student’s rehabilitation plan from admission to discharge.
• Document service provision and maintain student records; coordinate with rehabilitation personnel and outside agencies.
• Address total rehabilitative needs by coordinating with community resources and partners.
