Job summary

The North Carolina Department of Health and Human Services (DHHS) is seeking a Casework Associate I (S) in Wayne County at WorkSource East Training Center. The role tutors students referred to EIPD by counselors, developing rehabilitation plans and coordinating vocational skills training from admission to discharge.

The Case Manager works with other rehabilitation personnel, agencies and the community to address total rehabilitative needs and document services for successful outcomes.

Qualifications

• Knowledge of or experience in case management.
• Knowledge of or experience working with/counseling people with disabilities.
• Knowledge of or experience working within a team setting.
• Excellent written and oral communication skills.

Responsibilities

• Develop and implement the student rehabilitation plan from admission to discharge.
• Provide case management and coordinate training across vocational skills programs.
• Document service provision and maintain student records.
• Collaborate with rehabilitation staff, external agencies and community partners to address total rehabilitative needs.
