Job summary

The North Carolina Department of Health and Human Services (DHHS) is seeking a Case Manager I for Wayne County. You will serve as the case manager for Employment and Independence for People with Disabilities, building rehabilitation plans and coordinating services for students referred by EIPD Counselors.

You will document services, maintain records, and work with other professionals and agencies to address total rehabilitative needs, ensuring smooth transitions through admission to discharge.

Qualifications

• Bachelor’s degree in a discipline related to the program.
• One year of experience in a related area may substitute education requirements.

Responsibilities

• Develop and implement the student’s rehabilitation plan from admission to discharge.
• Document service provision and maintain student records.
• Collaborate with rehabilitation personnel, outside agencies, and the community to address needs.
