Job summary

ServiceSource seeks a dedicated Case Manager to support a multi-year grant-funded program that helps disconnected youth and adults with disabilities transition into competitive, integrated employment through comprehensive family supports.

As part of the support team, you will link participants to essential community resources, coordinate referrals and applications, and develop care plans while monitoring changes to ensure ongoing support and progress toward self-sufficiency.

Qualifications

• Bachelor’s degree in social work, psychology, or human services (required).
• Master’s degree in related field preferred.
• 2–5 years of experience with individuals with disabilities; TANF experience preferred.
• Valid driver’s license and reliable transportation for work travel.
• Annual background checks and required screenings per regulations.

Responsibilities

• Conduct comprehensive assessments to identify support needs and set individualized/family goals.
• Develop, implement, and update person-centered plans and coordinate referrals.
• Link participants to vital community resources and navigate complex systems.
• Monitor progress and address barriers to employment and independence.
• Build and maintain relationships with community partners and stakeholders.
• Record progress and participate in team meetings.
