Responsibilities

• Provides home-based care coordination and develops individualized care plans.
• Monitors member progress and updates plans with the Interdisciplinary Care Team.
• Verifies eligibility for waiver services and documents findings in the Care Management System.
• Travel throughout Cook County to assess and support members.
