TEKsystems in Detroit is seeking compassionate Analysts to support a statewide disability benefits outreach initiative. You will perform case management, advocacy, and administrative support, guiding residents through SSDI/SSI applications and gathering required documentation.

You will meet clients at community centers, educate on programs, assist with online and paper applications, and coordinate with healthcare providers and agencies to obtain records.

Formación

• Minimum 1 year experience in case management, social work, community outreach, community health work, benefits navigation, human services or rehabilitation services.

Responsabilidades

• Conduct intake assessments with individuals seeking SSDI and SSI benefits.
• Meet clients at community centers and designated locations within assigned regions.
• Educate individuals about SSDI/SSI programs, requirements and applications.
• Assist clients in completing online and paper applications.
• Gather, organize and maintain medical, employment and supporting documentation.
• Coordinate with healthcare providers, rehabilitation facilities, community organizations and government agencies to obtain records.
• Track application status and provide ongoing guidance through the determination process.
• Identify missing documentation and work with clients to address deficiencies.
• Maintain accurate case notes, records and reports in designated systems.
• Use case management systems to document activities and manage caseloads.
• Build trusting relationships with clients while maintaining confidentiality and boundaries.
• Participate in team meetings, training sessions and program improvement initiatives.
