Requirements include a Bachelor's degree (Master's preferred) and LSW/CSW licensure, with experience in healthcare settings and in nursing home or rehab environments. Strong communication and regulatory knowledge are essential for success.

Qualifications

• Bachelor's degree required; Master's degree strongly preferred.
• Current LSW/CSW licensure required.
• Experience as a social worker in a healthcare setting is required.
• Experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, standards, and guidelines governing nursing care facilities.

Responsibilities

• Provide leadership and oversight of social services programs to support residents and families.
• Ensure services comply with federal, state, and facility regulations.
• Collaborate with interdisciplinary teams to support residents’ care planning.
• Offer counseling, support, and advocacy for residents and families.
• Serve as liaison between residents, families, staff, and external agencies.
