Ultimate Healthcare Center is seeking a compassionate Director of Social Services to lead the social work team, assess residents' psychosocial needs, and coordinate care planning. You will guide transitions, support families, and promote quality of life.

The ideal candidate is a Maryland-licensed LSW or LCSW with SNF experience, excellent communication, and the ability to collaborate across disciplines. This on-site leadership role offers competitive pay and benefits.

Formación

• Must hold LSW or LCSW licensure in Maryland.
• Experience in a skilled nursing facility is required.
• Excellent communication and interpersonal skills.

Responsabilidades

• Conduct comprehensive psychosocial assessments and develop personalized care plans.
• Offer counseling and emotional support to residents and their families.
• Coordinate discharge planning and transitions of care to ensure seamless transitions.
• Serve as a liaison between residents, families, and care teams to ensure effective communication.
• Assist with admissions and resident adjustment to promote a smooth transition.
• Ensure compliance with state and federal regulations to maintain the highest standards of care.
