The Lodge at Mills River Health and Rehabilitation in Mills River, NC is seeking a Director of Social Services to lead our social work team and ensure compassionate, person-centered care for residents.

You will assess patient needs, develop care plans, coordinate referrals, and maintain accurate documentation while fostering a supportive, collaborative environment with staff, residents, and families.

Qualifications

• Previous experience in social work, with a focus on short term and long term care patients.
• Compassionate and caring demeanor.
• Ability to build rapport with staff, patients, and their loved ones.
• Strong leadership qualities.
• Excellent written and verbal communication skills.

Responsibilities

• Assess nature of patients' situations by interviewing and reviewing personal history.
• Develop and execute individual treatment plans/care plans.
• Determine appropriate milestones to gauge patient progress.
• Communicate patient updates to all relevant parties.
• Facilitate referrals to other healthcare professionals and programs as needed.
• Maintain accurate documentation.
