Job summary

Brighton/Suncrest Hospice seeks a Director of Social Services to provide overall direction for social services, lead the social work team, and ensure compliance with CHAP standards. The role emphasizes high-quality care, proper documentation, and strategic planning to meet patient and family needs.

The position requires an MSW, LCSW certification, and at least three years of management experience in home care settings.

Qualifications

• Master's degree in Social Work.
• Must be an LCSW.
• At least three years of management experience, preferably in home care operations.
• Ability to supervise and direct professional and administrative personnel.
• Good communications and public relations skills.
• Knowledge of corporate business management, governmental regulations and CHAP standards.
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• Recruits, selects, orients, and manages members of the social service team.
• Supervises assessment, planning, implementation, and evaluation of clinical social services.
• Ensures proper documentation of social services in clinical records per regulations.

Details

The Director of Social Services works to provide overall direction to all aspects of the provision of patient social services including managing the members of the social work team. Establishes, implements, and evaluates goals and objectives for social services to meet and promote the standards of quality and contribute to the organizations mission and philosophy.

• Directs all daily patient social service operations including providing direct oversight of the establishment and implementation of social service
• Ensures compliance with all state, federal, and CHAP social service regulatory
• Directs the implementation of improved work methods and procedures to ensure social services are delivered in accordance with local and state regulations
• Ensures seamless home care service delivery by providing direct oversight of coordination of care with other clinical service
• Assists the Executive Director/Administrator in the preparation of an annual budget for the social service department and monitors allocation of resources according to budgetary
• Recruits, selects, orients, and directly manages members of the social service
• Supervises the assessment, planning, implementation, and evaluation of social service clinical care by members of the social work
• Assures proper documentation of social services in clinical records in compliance with state, federal, and CHAP
• Provides leadership in strategic planning including identifying opportunities for additional or improved services to address unmet customer needs
• Maintains comprehensive working knowledge of the field of social work and shares information with appropriate organization
• Maintains comprehensive working knowledge of Brighton/Suncrest Hospice contractual relationships and ensures that social services are delivered according to contract

Qualifications

• A Master's degree in Social Work. Meets personnel qualifications stated in §418.114(b)(3)
• Must be an LCSW
• At least three (3) years management experience preferably in home care operations
• Demonstrates an ability to supervise and direct professional and administrative personnel
• Ability to deal tactfully with customers and the community
• Knowledge of corporate business management, governmental regulations and CHAP standards
• Demonstrates good communications and public relations skills
• Demonstrates autonomy, organization, assertiveness, and flexibility and cooperation in performing job responsibilities
