Zusammenfassung

Brighton/Suncrest Hospice in Walnut Creek, CA seeks a compassionate Hospice Social Worker to join a CHAP-certified team providing psychosocial support to patients and families during end-of-life care.

You will assess needs, counsel and educate families, advocate for access to resources, and participate in IDT meetings, while maintaining accurate documentation and contributing to a culture of communication and growth.

Qualifikationen

• Master of Social Work degree that complies with CHAP and state of residence regulations.
• Assess patients’ and families’ psychosocial, emotional, spiritual, and socioeconomic needs and incorporate findings into the individualized plan of care.
• Counsel patients and families while providing emotional support, crisis intervention, and guidance as they navigate illness, loss, grief, and significant life transitions.
• Educate patients and families about available community resources and connect them with appropriate programs, services, and support.
• Advocate for patients and families to identify barriers to care and the hospice team to develop appropriate solutions to ensure their needs, preferences, values, and goals are heard and addressed.
• Participate in interdisciplinary team (IDT) meetings and collaborate with physicians, nurses, chaplains, aides, facility staff, and other members of the care team.
• Maintain accurate, thorough, and timely documentation in accordance with hospice standards and regulatory requirements.

Our Hospice Social Workers provide compassionate psychosocial support to patients, families, referring source and team members as they navigate the emotional, social, and practical challenges of serious illness and end-of-life care.
