Job summary

Cura of Le Sueur seeks an experienced MDS Coordinator to join our nursing leadership team. You will oversee the MDS and RAI processes, ensuring accurate assessments, care planning, and compliant documentation across the facility.

The role requires strong clinical judgment, attention to detail, and knowledge of Medicare/Medicaid reimbursement. You will collaborate with nursing, therapy, social services, and physicians to support high-quality resident care.

Qualifications

• Current and active RN license in Minnesota.
• Experience coordinating MDS/RAI processes is preferred.
• Strong attention to detail and documentation accuracy.
• Ability to collaborate with interdisciplinary teams and meet regulatory requirements.

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state requirements and timelines.
• Manage the RAI process and maintain an accurate MDS schedule.
• Ensure assessments are accurate and submitted within required timeframes.
• Collaborate with nursing, therapy, social services, and other departments for care planning.
• Review documentation for coding accuracy and reimbursement classifications.
• Educate staff on MDS requirements and HIPAA compliance.
• Support audits, surveys, and requests for supporting clinical information.
