PRC Baker Places in San Francisco is a nonprofit delivering legal advocacy, workforce development, financial services and residential treatment. The Director of Intake & Placement coordinates client flow across Baker Places programs and interviews clients in settings including jails and hospitals to ensure appropriate care levels.

This role partners with Residential, Respite, Leland House and Case Management Directors to maintain high standards of client care and staff support, while supervising

Qualifikationen

• Active California license as LCSW/LMFT/Psy.D or equivalent.
• Minimum 2 years in adult/older adult mental health settings.
• Ability to obtain jail clearance.

Aufgaben

• Conducts and documents client interviews in various settings.
• Reviews and compiles clinical information for program placement.
• Maintains accurate documentation and timely reporting.
• Supervises the Intake Specialist.
• Assesses clinical appropriateness for program entry.
• Maintains agency data records and HIPAA compliance.
• Develops partnerships with community agencies and funders.
• Coordinates flow of clients within the care continuum.
• Participates in LPHA on-call rotation.
