Healthcare facility seeks a Director of Case Management to lead care-management, utilization management, discharge planning, social work, and psychosocial interventions. Role focuses on safe, effective transitions of care, patient outcomes, satisfaction, throughput, and alignment with benchmarks.

Strategic and operational plans, along with management of clinical, operational, and financial metrics, are key parts of the position.

Qualifications

• Bachelor's in nursing and Master's in health-related field required.
• 2+ years of care management in an acute-care setting.
• 2+ years of healthcare leadership related to acute-care case management.
• Strong knowledge of acute-care case management principles.
• Understanding of evidence-based practice and care-transition models.
• Knowledge of the care continuum and transition-of-care strategies.

Responsibilities

Provides strategic and operational leadership for all care-management functions across the acute-care facility, including case management, utilization management, discharge planning, social work services, and psychosocial interventions that support safe, effective, and sustainable transitions of care. Accountable for patient outcomes, satisfaction, throughput, and care continuum utilization while driving performance that meets or exceeds regional and national benchmarks. Develops and executes specific strategic and operational plans, manages key clinical, operational, and financial metrics, and ensures services remain patient-centered and aligned.

Qualifications:

• Bachelor's degree in nursing AND a Master's degree in a health-related/science field.
• 2+ years of care management experience in an acute-care setting.
• 2+ years of healthcare leadership or management experience related to acute-care case management.
• Strong knowledge of acute-care case management principles and practices.
• Understanding of evidence-based practice and care-transition models.
• Knowledge of the care continuum and effective transition-of-care strategies.
