Responsibilities

• Directs, plans, implements and evaluates care coordination across the continuum.
• Lead, develop and mentor RN case management and social work services teams.
• Develop strategic 6–12 month plan and drive programmatic improvement.
• Collaborate with physician leadership to implement patient-focused interventions.
• Monitor utilization, LOS, and length of stay reduction strategies with data-driven insights.
• Ensure regulatory compliance and liaison with third-party payers for coverage.
• Oversee discharge planning and transitions of care for complex patients.
