You will partner with nursing, therapy, and social work teams to coordinate care and maintain robust relationships with hospitals and referral partners.

You will oversee documentation, monitor census trends, and drive occupancy goals while upholding high-quality care and compliance.

• Collaborate with nursing, therapy, and social work teams to support seamless transitions.
• Maintain accurate and timely documentation of all admissions activity.
• Develop and maintain relationships with local hospitals, case managers, and community organizations.
• Monitor census trends and contribute to occupancy planning.
• Represent the facility professionally in outreach and marketing efforts.
