Job summary

Lynchburg Health & Rehabilitation Center in Lynchburg, VA is seeking an experienced Director of Discharge Planning and Social Services to coordinate patient transitions home and ensure safe, smooth discharges.

You will lead discharge meetings, arrange post-acute services, and collaborate with case managers, therapists, and families to address social needs and optimize bed availability. A 4-year degree in a human services field and 1+ year in healthcare social work are required.

Qualifications

• 4-year degree in Social Work, Psychology, Sociology, Education, Counseling, Criminal Justice, or other human services related degree.
• At least 1 year of Social Work experience in a healthcare setting.
• Strong communication and collaboration skills.

Responsibilities

• Research, identify, and arrange external services for residents such as Chaplaincy, Advocates, Responsible Parties, and optimal care environments.
• Schedule discharge meetings and coordinate home visits, home care, therapy according to resident needs.
• Directs timely and efficient five-day discharge planning meetings for all newly admitted residents.
• Assesses patient's support systems and home situation to assist in decision-making regarding discharge plan and needs.
• Facilitates effective bed management by strategic evaluation and internal arrangement to maximize availability of transitional and extended care services.
• Meet with and assist residents with social issues such as conflict resolution, problem-solving, and care coordination.
• Conduct customer satisfaction surveys.
