Job summary

Lynchburg Health & Rehabilitation Center in Lynchburg, Virginia, is seeking an experienced Director of Discharge Planning and Social Services to coordinate residents' discharge, including supplies, transportation, and services to ensure safe transitions home.

You will lead discharge meetings, assess home support, and collaborate with care teams. A 4-year degree in a human services field and 1+ year in social work within healthcare are required; 2+ years in discharge planning preferred.

Qualifications

• 4-year degree in Social Work, Psychology, Sociology, Education, Counseling, Criminal Justice, or related human services field.
• At least 1 year of Social Work experience in healthcare settings.
• Discharge planning and social services experience preferred.

Responsibilities

• Coordinate discharge planning including supplies, transportation and services.
• Schedule discharge meetings and home visits; lead five-day discharge planning meetings.
• Assess patient’s home and support systems to aid in discharge decisions.
• Facilitate bed management to maximize availability of transitional and extended care.
• Meet with residents to address social issues and care coordination.
• Conduct customer satisfaction surveys.
