Prairie Community Services is seeking a Direct Support Provider on-site in Fergus Falls, MN. The role involves supervising and assisting individuals in daily living activities, meals, recreation, and community orientation, with medication management support as needed.

Starting wage ranges from $16.27 to $18.27 per hour. A DHS background clearance, valid driver’s license, and English proficiency are required, along with the ability to document daily progress and follow person-centered plans.

Qualifications

• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language.
• Read and follow each person served’s program plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Assist with daily living activities, recreation, community orientation, meals, and grooming.
• Ensure accurate documentation and reporting of daily progress.
