Job summary

Prairie Community Services is seeking a Direct Support Provider for the Arlington Home in Fergus Falls, MN. This on-site, full-time role involves supervising and assisting individuals with daily living activities, meals, and community integration.

The position requires English fluency, a valid driver’s license, and the ability to follow person-centered plans. Starting wage is $16.27–$18.27 per hour with benefits and DHS background clearance.

Qualifications

• Minimum of 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Assist with daily living activities and meal preparation.
• Support recreation and community orientation.
• Administer medications as directed and document outcomes.
