Oak Hill, Connecticut's largest private provider of disability services, is hiring a Direct Support Professional in Rocky Hill. You will support individuals with daily living, health monitoring, and community engagement, including transportation and meal assistance.

We offer paid training, DDS certification within 90 days, and career mobility after six months. Benefits include medical, dental, vision, 403(b) with company contribution, and generous PTO.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver’s license with the ability to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including occasional shifts beyond schedule.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene needs.
• Provide continence care as needed.
• Encourage independence and skill development.
• Monitor health information and vital signs as required.
• Administer medications and maintain medication records as directed.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and assist during meals according to dietary needs.
• Assist with mobility and transfers using approved equipment.
• Document daily activities and health information in the EHR.
• Follow emergency procedures and safety protocols.
