OneWell Health Care is seeking a dedicated Caregiver / Direct Support Professional (DSP) to support individuals with IDD and Autism in Northampton, PA, in home and community settings. You will work under supervision to implement ISP/IPP, promote independence, and assist with daily living activities, health and safety monitoring, and community participation.

Applicants should have a high school diploma or equivalent, and experience with caregiving or home health is preferred.

Qualificações

• High school diploma or equivalent required.
• Experience in caregiving or home health preferred.
• Ability to use electronic documentation and EVV systems.
• Follow written care plans and documentation guidelines.

Responsabilidades

• Provide hands-on, person-centered support in home/community settings.
• Assist with daily living activities, meals, mobility, and medication as authorized.
• Implement behavior support strategies and monitor health, safety, and well-being.
• Maintain structured routines and assist with transportation and community participation.
