Qualifications

• High school diploma required; further education in human services or related field is a plus.
• Excellent problem-solving and interpersonal skills.
• Ability to work independently and collaboratively.
• Proficiency with Microsoft applications and general computer skills.

Responsibilities

• Person-Centered Support: provide individualized support to help individuals lead self-directed lives within their communities.
• Skill Development: support growth aligned with PCP/ISP.
• Community Engagement: facilitate opportunities to engage in social networks and activities.
• Daily Living Assistance: coaching for ADLs and IADLs.
• Health & Safety: ensure clean, safe environments and follow care plans.
• Adaptability: adjust approaches as needs evolve.
