Job summary

Heartshare Wellness LTD is seeking a Direct Support Professional (DSP) in New York. The role focuses on implementing person-centered services, ensuring that individual plans and goals are executed in both program and community settings.

You will support skill development, socialization, and participation across activities, while safeguarding confidentiality and coordinating with families and providers. The DSP will transport individuals, assist with community activities, document progress, and

Qualifications

• High School Diploma, GED or CNA strongly preferred but not required.
• One year or more experience with individuals with developmental disabilities preferred.
• NYS driver’s license preferred but not required.

Responsibilities

• Implement individuals’ plans aligned with services, goals and outcomes in program and community settings.
• Support individuals in skills development, socialization, and task participation.
• Promote inclusion and foster friendships with neighbors, co-workers and peers.
• Maintain communication with family, advocates and service providers as needed.
• Transport individuals or assist with public transportation.
• Plan recreational activities and participate in PPT meetings.
• Document medical, behavioral and service-related data accurately and timely.
• Complete all written reports and documentation on time.
• Provide supervision and training per LOS requirements.
• Ensure confidentiality and privacy of all staff and individuals.
• Meet or exceed the DSP Core Competencies Standards by year end.
