Job summary

Heartshare is hiring a Direct Support Professional (DSP) in New York to implement person-centered services for individuals with developmental disabilities. The role includes promoting community inclusion, documenting outcomes, and coordinating with families and providers.

Starting hourly base rate is $19 with potential additional compensation after hire based on certifications and performance. Benefits and a bi-annual discretionary bonus may apply as part of total compensation.

Qualifications

• AMAP, CPR/First Aid and SCIP certifications may be required.
• One year or more experience with individuals with developmental disabilities preferred.
• NYS driver’s license preferred but not required.

Responsibilities

• Implement individuals’ plans and activities aligned with goals and outcomes.
• Support individuals in skills development, socialization, and tasks across settings.
• Promote inclusion and community engagement for individuals.
• Maintain communication with family, advocates, and service providers.
• Transport individuals or assist them with public transportation.
• Document information accurately in medical, behavioral and service records.
• Complete reports in a timely manner.
• Provide supervision and ensure staff are trained on LOS per person.
• Safeguard confidentiality and privacy of staff and individuals.
• Meet or exceed DSP Core Competencies by year end.
