Job summary

Lynn & Darla, LLC in Solano/Napa County is seeking a Direct Support Professional to assist people with developmental and physical disabilities in their homes. You will implement the Individual Program Plan and provide guidance and advocacy to individuals, coordinating with a support team to help them live as independently as possible.

This role requires CPR/First Aid, Crisis Prevention Intervention and Person Centered Training (paid), plus a valid driver's license.

Qualifications

• Preferred 1-year direct support experience with people with developmental, physical or psychiatric disabilities or transferable skills
• Driver's license and reliable transportation
• Must have and maintain current CPR/First Aid Certification and present upon hire
• Crisis Prevention Intervention paid training
• Person Centered Training paid training

Responsibilities

• Assure that individuals served are provided with assistance in all aspects of their daily living and supports to live a full active life of their choice
• Implement and follow through on all aspects of the Individual Program Plan (IIP) for people served
• Provide advice, guidance, and support to individuals regarding personal issues and life choices
• Advocate for people served
• Liaise with the individual’s team
• Attend Individual Program Plan meetings (annual and interim)
• Attend weekly team meetings
• Ensure data collection and daily documentation is completed
• Ensure individuals have access to nutritious food they enjoy
• Report symptoms of illness to the physician of choice
• Arrange transportation when needed for community activities
• Accompany people to activities
• Assist people to budget and manage finances
• Respect the human and civil rights of people served
• Assist with exterior and interior home maintenance as needed
• Coordinate with landlords and repair services and report results
• Communicate with day program services politely
• Encourage meal preparation as per individual schedule
• Prepare administrative reports and documents
• Complete required training and keep it current
• Attend Regional Center audits as requested
• Adhere to Injury & Illness Prevention Program
• Perform other duties as assigned
