Job summary

Evergreen Life Services seeks compassionate Direct Support Workers to maintain safe, therapeutic environments for individuals with disabilities. You will assist with daily living, medications, budgeting, meals, and community activities, while following individualized plans of care.

This role involves documentation, behavior support, transportation, and collaboration with a caring team. A good driving record, reliable transportation, and a genuine desire to help people matter.

Qualifications

• Must be at least 21 years old due to auto liability coverage.
• Able to read, write and communicate verbally in English; able to read instructions, perform records and write reports.
• Meet and maintain all requirements of applicable state agencies, including good standing with state abuse and neglect registry.
• Ability to learn basic computer skills (log in/out, input data, approve timesheet).
• Prior experience as a direct support worker preferred.

Responsibilities

• Maintain a healthy, safe, and therapeutic environment for individuals served and follow their plans of care.
• Assist individuals with medication and treatment plans as needed.
• Assist with care and cleaning of home, including reporting maintenance needs.
• Assist with money management including budgeting, banking, and shopping.
• Assist with daily life activities: meals, reading, phone use, community services, housekeeping, clothing care, self-care, personal hygiene.
• Make food and non-food purchases for the home when requested; prepare meals.
• Ensure home and grounds are maintained as requested.
• Complete required documentation per state agencies and Evergreen policies.
• Implement behavioral programs and assist with goals in care plans.
• Identify potential behavior triggers and defuse or redirect as needed for safety.
• Provide transportation in company or personal vehicle as needed.
