Job summary

Lynn and Darla Supported Living is seeking a dedicated individual to provide direct support services in clients' homes under supervision. The role emphasizes assisting with daily living, following the Individual Program Plan, and advocating for people served.

You will coordinate with a team, maintain documentation, and help with meals, budgeting, and transportation as needed. The position requires at least 1 year of direct support experience (preferred), current CPR/First Aid certification, and

Qualifications

1 year direct support experience with people with behavior challenges, medical involvement or psychiatric diagnosis is preferred.
Must have and maintain current CPR/First Aid Certification.
Must obtain Lynn & Darla General Orientation & Handbook Training and CPI within 30 days of employment.

Responsibilities

Assure individuals served receive assistance in all aspects of daily living to live a full life of their choice.
Implement and follow through on the Individual Program Plan (IIP) for individuals served.
Provide advice, guidance and support for personal issues and life choices.
Advocate for people served and act as liaison to the individual’s team.
Attend Annual and interim IIP meetings and weekly team meetings.
Ensure data collection and daily documentation are completed and up to date.
Ensure access to nutritious food and transportation as needed.
Assist individuals with budgeting and maintaining their home exterior/interior.
