Job summary

Responsibilities include implementing Individual Plans, maintaining data and documentation, attending meetings, and ensuring a clean, safe environment. Must be 18+, with a high school diploma or GED.

Qualifications

• Must be at least 18 years old.
• High School diploma, GED or equivalent.

Responsibilities

• Support participants to achieve the highest level of independence possible.
• Provide input and implement the participant’s Individual Plan.
• Follow organizational guidelines to promote health and safety.
• Monitor, respond to and report issues to the Case Coordinator or supervisor.
• Provide personal care and assistance with daily living activities as needed.
• Accurately complete all participant-related data and documentation by the assigned deadline via a web-based platform.
• Submit timesheets, leave requests, credentialing documents, and training paperwork by deadline via FoC platforms.
• Attend and participate in department meetings and training.
• Maintain a clean, hygienic, safe environment.
• Respond to emergencies per FoC procedures.
