Qualifications

• Must be at least 18 years old.
• High School diploma, GED or equivalent.
• Exercise patience and understanding with participants of varying abilities.
• Be culturally respectful with diverse populations.
• Communicate effectively and read in the workplace language.
• Maintain HIPAA confidentiality and discretion.
• Proficient with computer/phone technology and standard software.
• Able to work with individuals at all levels within the organization.

Responsibilities

• Support participants to achieve the highest level of independence.
• Implement the participant's Individual Plan.
• Promote health and safety per organizational guidelines.
• Monitor and report issues to the Case Coordinator or supervisor.
• Provide personal care and daily living assistance as needed.
• Complete participant data and documentation by deadline.
• Submit timesheets and required paperwork on time via FoC platforms.
• Attend department meetings and training as required.
• Assist in maintaining a clean, hygienic, safe environment.
• Respond to emergencies per FoC procedures.
• Report safety concerns to the Program Manager.
