Qualifications

• Must be at least 18 years of age.

• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to pass background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain certifications.
• Commitment to safety and professional work environment per Oak Hill policies.
• Commitment to continuity of care, including occasional shift overtime if needed.

Responsibilities

• Assist with daily living activities including personal care, grooming, bathing, dressing, and hygiene.
• Assist with continence care as needed.
• Encourage independence and skill development.
• Monitor health information and vital signs when required.
• Administer medications and maintain medication records.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and snacks per menus and dietary needs; assist during meals.
• Assist with mobility and transfers using approved equipment.
• Document daily activities and health information in the EHR; follow care plans.
• Respond to emergencies and apply First Aid/CPR when required.
• Communicate effectively with team, individuals served, families, and stakeholders.
• Participate in meetings, trainings, and ongoing professional development.
