Oak Hill in Lebanon, CT is hiring a Direct Support Professional for 27 weekly hours, primarily overnight Mon-Wed 11pm-8am. You will support individuals with daily living, health monitoring, and community activities, transporting clients as needed.

Responsibilities include medication administration under directives, documentation in the EHR, and implementing safety protocols. Required: 18+, HS diploma/GED, valid driver's license, ability to pass background checks and trainings.

Qualifikationen

• Must be 18 years of age or older.
• High school diploma or GED required.
• Valid driver’s license with the ability to transport individuals in agency vehicles.
• Able to pass background checks, fingerprinting, and pre-employment health screenings.
• Complete required trainings and maintain certifications.
• Commitment to safe, professional work environment per Oak Hill policies.
• Willing to stay beyond scheduled shifts to ensure coverage.

Aufgaben

• Assist with personal care, grooming, bathing, dressing, and hygiene per plans.
• Monitor health information and document vital signs when required.
• Administer medications per training and maintain records in the EHR.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and assist with mealtimes per dietary needs.
• Support mobility and transfers using approved techniques and equipment.
• Document daily activities, health data, and incidents in the EHR.
• Follow emergency procedures and safety protocols; apply first aid/CPR when needed.
• Communicate effectively with team, individuals served, and families.
