Job summary

REM Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. This position offers direct support within residential or program-based settings, typically serving 4–6 individuals per unit and up to 10 in periodic programs.

As a non-exempt, hourly role, you will assist with daily living activities, health maintenance, and transportation, while documenting progress and ensuring safety.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: personal care, housekeeping, recreation, transportation, community integration, shopping, and other activities of daily living.
• Accurately documents progress and activity; reviews records and logs to stay informed of changes in service plans; maintains confidentiality.
• Maintains healthy and professional relationships with individuals, families, guardians and case managers; greets visitors; adheres to Sevita Customer Service Standards.
• Reports any instance of alleged abuse or neglect; reports medical and other incidents per Sevita policy and external requirements.
• Maintains confidentiality and respects the rights of persons served; follows health care directives.
