Job summary

REM Community Services, a part of the Sevita family, is seeking a Direct Support Professional to provide direct support in residential settings. You will assist 4–6 individuals per program, delivering care per individual service plans and participating in socialization and health-related activities.

Responsibilities include documentation, confidentiality, and collaboration with families and case managers. This role is non-exempt and hourly, with potential both full-time and part-time shifts,

Qualifications

• High school diploma or equivalent required/preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, and other daily living tasks.
• Accurately document progress and activity; review records and logs to stay up-to-date with changes in service plans; maintain confidentiality.
• Maintain healthy and professional relationships with individuals, families, guardians and case managers; follow Sevita Customer Service Standards.
