Job summary

Horizon Health and Wellness, Inc. in Tempe, AZ seeks a dedicated Direct Support Professional to assist individuals with intellectual and developmental disabilities, promoting independence and community participation.

You will provide care, supervise activities, and help participants stay safe while supporting daily living skills. Training is provided; evenings, weekends, and holidays may be required.

Qualifications

• High School diploma or GED.
• Must be 21 years of age or older.
• Valid driver’s license with acceptable driving record and reliable transportation.
• Ability to operate computers and electronic clinical record keeping.
• Able to obtain fingerprint clearance and pass background checks.
• Willingness to work nights, weekends, holidays.

Responsibilities

• Provide care services to individuals with intellectual and developmental disabilities.
• Support community integration/inclusion for participants and families.
• Ensure health, safety, and well-being of residents through proper supervision.
• Develop opportunities for participation in community activities and document activities in required records.
