Job summary

Horizon Health and Wellness in Phoenix, AZ is seeking a compassionate PRN Direct Support Professional to assist individuals with intellectual and developmental disabilities. You will help with community integration, provide guidance to participants and families, and ensure safety and wellbeing in residential settings.

Shifts are available on an as‑needed basis during the holiday season, with possible continued PRN opportunities afterward.

Qualifications

• High School diploma or GED.
• Must be 21 years of age or older.
• Possess valid driver’s license with acceptable driving record, MVD report and reliable transportation.
• Ability to prepare required paperwork to support and document activities in the contractually required timelines.
• Ability to type and working knowledge with computers and electronic clinical record keeping.
• Ability to complete certification and trainings for First Aid, CPR, Article 9, Medication, and Prevention/Support, Direct Care Worker and any other required training. Training provided if needed.
• Able to operate independently with minimal oversight.
• Demonstrated skills to ensure respectful communication to participants and coworkers.
• Must obtain fingerprint clearance card and pass a background check along with Central Registry Clearance.
• Willingness to work nights, weekends, holidays and a varied schedule.

Responsibilities

• Provide essential care and support services to individuals with intellectual and developmental disabilities.
• Assist with community integration and inclusion, guiding individuals and their support networks, and create meaningful opportunities for participation in community activities.
• Ensure health, safety and overall well‑being of individuals living in a residential setting by providing proper supervision and support.
• Support shifts as needed during the holiday season with potential opportunities afterward.
