Horizon Health and Wellness, Inc. seeks compassionate Direct Support Professionals to assist individuals with developmental disabilities in community and residential settings. You will support daily living, promote social inclusion, and help participants engage in local activities.

Role requires documentation in electronic records, ability to follow contracted timelines, and completion of required trainings such as First Aid and CPR. Nights, weekends, and varied schedules are expected.

Competenze

• High School diploma or GED required.
• Must be 21+ years old and have a valid driver's license.
• Ability to document activities in required timelines.
• Ability to use computers and electronic clinical records.
• Ability to obtain training for First Aid, CPR, and Direct Care Worker certifications.
• Ability to work independently with minimal supervision.
• Willingness to work nights, weekends, holidays.
• Fingerprint clearance card and background checks required.
• Bilingual English/Spanish or English/Sign Language preferred.
• Two years of experience in habilitation/rehabilitation or community integration preferred.

In addition, they develop opportunities for persons with disabilities to participate in community activities while ensuring the health, safety, and well-being of residential participants living in the facility through proper supervision.Qualifications RequiredHigh School diploma or GEDMust be 21 years of age or older.Possess valid driver’s license with acceptable driving record, MVD report and reliable transportation.Ability to prepare required paperwork to support and document activities in the contractually required timelines.Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.Ability to successfully complete certification and trainings for First Aid, CPR, Article 9, Medication, and Prevention/Support, Direct Care Worker and any other required training.
