Job summary

Horizon Health And Wellness in Tempe, AZ is seeking compassionate Direct Support Professionals to provide care and support to individuals with intellectual and developmental disabilities. You will help with health, safety, and community integration, and assist residents in daily activities within the facility.

Responsibilities include personal care, documentation, training, and working varied shifts, including nights, weekends, and holidays.

Qualifications

• Must have a High School diploma or GED.
• Valid driver’s license may be required for transportation duties.
• Typing and computer skills to complete documentation and record-keeping.
• Ability to prepare required paperwork within contract timelines.
• Willingness to work nights, weekends, holidays, and a varied schedule.

Responsibilities

• Provide personal care and support to individuals with intellectual and developmental disabilities.
• Guide community integration and inclusion efforts for participants, families, and stakeholders.
• Develop and facilitate opportunities for residents to participate in community activities.
• Maintain proper supervision and ensure safety and well-being of residents in the facility.
• Prepare and complete required paperwork and documentation within contractual timelines.
• Use computers and electronic record‑keeping systems effectively.
• Complete mandatory certifications and trainings (First Aid, CPR, Article 9, Medication, Prevention/Support, Direct Care Worker).
• Work nights, weekends, holidays, and maintain a varied weekly schedule.
