Job summary

CareLink Community Support Services is seeking dedicated Direct Support Professionals to assist individuals with disabilities in community living settings. You will assess needs, implement recovery-oriented plans, and support daily activities including meals, hygiene, and transportation.

You will document progress and collaborate with teams to promote independence and well-being. Equal opportunity employer; eligible for robust benefits and PTO.

Qualifications

• Required education: high school diploma or GED.
• Two to three years of work experience in human services or related field.
• Valid driver’s license and CPRP/RPRP preferred.

Responsibilities

• Assess and support individuals' needs, preferences, and goals.
• Develop and implement service plans reflecting residents' recovery goals.
• Assist with daily living activities, meal prep, housekeeping, and outings.
• Document daily progress and staff interventions in writing.
• Assist with medication management and PRN medications.
• Encourage positive relationships with family and community resources.
• Promote personal hygiene, health, and safety for residents.
• Drive agency vehicles to support individuals and programs.
• Ensure safety procedures and emergencies are handled appropriately.
