Job summary

CareLink Community Support Services in West Chester, PA is seeking compassionate staff to help individuals with disabilities live with dignity and independence. You will support daily living activities, document progress, and promote recovery and self-determination in a community setting.

Responsibilities include assisting with meals, housekeeping, transportation, and accessing community resources while maintaining safety and respectful communication.

Qualifications

• Required Education: High School Diploma or GED.
• Preferred Education: Bachelor's Degree.
• Field of study: Human Services or related field.
• Work Experience (preferred): 1 to 2 years.
• Life Experience: Working in Human Services field or relevant life experience.
• Certifications, Licenses, Registrations Required: Valid Driver’s License, CPRP/RPRP Certification is preferred, Certified Peer Specialist is preferred.

Responsibilities

• Develop and implement service plans reflecting the individual’s preferences, goals, lifestyles, and interests.
• Assess, teach, evaluate, and supervise individual’s daily living, meal planning, housekeeping, career needs, and social, recreational, and community activities.
• Document progress daily reflecting recovery and progress on goals and staff interventions.
• Assist and oversee individuals with medication regimen including PRN medications.
• Promote recovery, wellness and self-determination in daily activities.
• Encourage and support individuals in building positive relationships with family and others.
• Maintain clean and safe living environments and participate in safety procedures.
• Assist with meal preparation and dietary considerations; perform food shopping as needed.
