VistaCare in Oshkosh, WI is hiring Direct Support Professionals (DSPs) to empower individuals with intellectual and developmental disabilities to live fulfilling, independent lives in a supportive community setting.

You will provide daily living support, implement ISPs and BSPs, assist with medication logging in EHR, support community engagement, and help maintain a safe, welcoming home environment. Full-time and part-time schedules with day, evening, and weekend shifts available.

Qualifications

• Age 18 years or older.
• Education: High School Diploma or GED.
• License: Valid Driver’s License with a clean driving record.
• Background Check: Must pass a caregiver background check.

Key Responsibilities

Daily Living Support: Assist individuals with personal hygiene, dressing, grooming, and meal preparation.

Plan Implementation: Carry out Individual Service Plans (ISPs) and Behavioral Support Plans (BSPs).

Medication & Logging: Safely administer medications and maintain accurate daily progress notes in electronic health records (EHR).

Community Engagement: Encourage personal choice, self-advocacy, recreational activities, and community involvement.

Environment: Maintain a clean, safe, and positive living environment for all residents.
