VISTA CARE in Fond du Lac, WI is seeking Direct Support Professionals (DSPs) to join a dedicated team. This full-time, non-exempt role focuses on personalized care, daily assistance, and empowering individuals to build independence in a safe, supportive home.

Pay ranges from $16.88 to $17.50 per hour under a blended premium-pay model. Benefits include health coverage options, a 401(k) plan, PTO, and access to daily pay.

Qualifications

• Must be 18 years of age or older.
• High School Diploma or GED required.
• Valid Driver's License and reliable transport.
• Must be able to pass a caregiver background check.
• Basic computer skills for daily electronic record-keeping.

Responsibilities

• Daily Care & Support: Assist individuals with daily living activities, including personal hygiene, dressing, and meal preparation.
• Plan Implementation: Follow and execute Individual Service Plans (ISP) and Behavioral Support Plans (BSP).
• Medication & Documentation: Safely administer medications and accurately log daily progress and observations in electronic health records (EHR).
• Community Engagement: Encourage personal choice, advocate for individual preferences, and support involvement in local community and recreational activities.
• Safety & Environment: Maintain a clean, safe, supportive, and positive living space.
