Job summary

Qualifications

• High School Diploma or GED (preferred).
• At least 18 years of age.
• Valid driver’s license.
• Pre-employment screens including background checks and verifications.

Responsibilities

• Support individuals in the community as part of waiver services.
• Assist with identifying and communicating needs; implement services according to each person’s Centered Plan.
• Accompany individuals to work, appointments, and community activities; provide transportation as needed.
• Promote independence by teaching daily living skills and facilitating decision-making.
• Document the care provided and support rights, confidentiality, and safety.
