Job summary

Oak Hill is seeking a Direct Support Professional in Watertown, CT. The position supports individuals with daily living, health, nutrition, mobility, and community inclusion. 40-hour schedule Monday through Friday, 3:00 PM to 11:00 PM, with a starting compensation of $20.39 per hour, and union clouding with overtime eligibility.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver''s license with ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.

Responsibilities

• Support individuals in daily living activities including personal care, grooming, bathing, dressing, and hygiene needs.
• Monitor and document health information; administer medications per training and nursing directives.
• Transport individuals to appointments and community activities using agency vehicles.
• Prepare meals and snacks according to menus and dietary requirements.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities, health information, progress notes, incidents, and med administration in the EHR.
• Follow emergency procedures and safety protocols; provide First Aid/CPR when required.
• Collaborate with team, participate in meetings and trainings.
