Responsibilities

• Assist adults with disabilities in daily living activities (household maintenance, meals, shopping, personal care).
• Accompany individuals to medical appointments and community activities.
• Follow Individual Support Plans and Behavior Support Plans to promote independence and safety.
• Communicate effectively with individuals, families, and team members to ensure quality care.
