Job summary

WellLife Network is seeking Direct Support Professionals to assist New Yorkers with developmental disabilities in homes and communities. The role focuses on promoting independence, dignity, and growth, with a strong emphasis on safety, medication administration, and daily living support.

Responsibilities include transportation, medication monitoring, data documentation, and collaborative goal planning with the management team.

Qualifications

• High School diploma or equivalency required.
• Valid New York State Driver’s License with 1 year of driving experience.
• Must be comfortable operating a large vehicle.
• Experience working with adults with intellectual/developmental or other behavioral health disabilities preferred.

Responsibilities

• Ensures activities support each person’s social responsibility, independence, dignity and growth.
• Implements goals to maximize independence including budgeting, grooming, nutrition planning, transport, and community resources.
• Safely operate agency vans/vehicles per driving policy.
• Complete AMAP, SCIP, CPR and First Aid certifications; employment conditional on passing exams.
• Coordinate evacuation and ensure safety in emergencies and drills.
• Administer medication and monitor self-administration when certified; document EMAR where applicable.
• Monitor and document progress on daily checklists, behavior plans and progress notes.
• Communicate information to staff and supervisors verbally and in writing.
• Meet with individuals and management to review progress and plan updates.
• Assist with evening/weekend recreation planning and activities.
• Provide ADL training and total-support tasks (e.g., diapering, ambulation, transfers).
• Attend meetings and trainings; act as role model; maintain confidentiality.
