Job summary

Continuum of Care is seeking a Direct Support Professional to assist individuals with intellectual disabilities in living independently and participating in community life. The role emphasizes respectful care, daily living support, and collaboration with care teams.

The DSP will document progress, coordinate transportation, and participate in planning to help each person reach personal goals while ensuring safety and dignity in all interactions.

Qualifications

• Bachelor’s or Associate degree preferred.
• High School Diploma or equivalent with related human services experience acceptable.
• Connecticut driver’s license with clean driving record required.
• CPR & First Aid, PMT certifications and DDS/CoC requirements completed.

Responsibilities

• Establishes rapport and professional relationships with individuals and families.
• Treats individuals with respect and dignity.
• Provides face-to-face hands-on support for ADLs and daily living.
• Delivers services per Individual Plan and positive behavioral supports.
• Encourages involvement in social and community activities.
• Provides companionship and problem-solving discussions.
• Supports attending medical, dental, and other appointments.
• Provides access to community resources via transportation.
• Contributes to Individual Plan development to achieve goals.
• Coordinates with community providers for quality care.
• Records daily data in Therap or designated systems.
• Attends staff and designated meetings.

Education
