Job summary

Continuum of Care is seeking a Direct Support Professional to assist individuals with intellectual disabilities in their own homes, supporting daily living and community involvement.

The DSP maintains respectful, dignified care, supports personal care, and helps individuals achieve goals through their plan and community resources. Evening, weekend, and overnight availability is required.

Qualifications

• Bachelor’s or Associate’s degree preferred; HS diploma accepted with relevant experience.
• Valid CT driver’s license with clean record.
• CPR & First Aid certification preferred; PMT and DDS/CoC requirements may apply.
• Computer skills to navigate electronic records and documentation.

Responsibilities

• Establish rapport and respectful care for individuals with disabilities.
• Provide face-to-face support for daily living and personal care.
• Assist with transportation to appointments and community activities.
• Record daily data in Therap or designated system.
• Attend staff meetings and complete assigned tasks.
