This position offers a 40-hour weekly schedule, weekend coverage may be required, a base rate of $20.39/hour with differential pay for specific schedules, and comprehensive training and development

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver’s license with ability to transport individuals in agency vehicles.
• Ability to complete required background checks and health screenings.
• Ability to complete required training programs and maintain certifications.
• Commitment to a safe, healthy, professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including occasional shift extensions.

Responsibilities

• Support individuals in daily living activities.
• Assist with personal care, grooming, bathing, dressing, and hygiene needs per plans.
• Provide continence care as needed.
• Encourage independence and skill development.
• Monitor health and document vital information as required.
• Administer medications and maintain medication records.
• Follow health care plans and support overall well-being.
• Transport individuals to appointments and community activities using agency vehicles.
• Encourage participation in community life and personal interests.
• Prepare meals and snacks according to menus and dietary needs.
