Prestige Home Care LLC is seeking a Direct Support Professional (DSP) to provide person-centered home and community-based services to individuals enrolled in Pennsylvania ODP waivers. You will help clients achieve independence, participate in the community, and improve health and quality of life by implementing each person's ISP.

The role requires working in homes and community settings with potential evenings, weekends, and holidays.

Qualifications

• High school diploma or GED required.
• Must be at least 18 years old.
• Ability to pass required criminal background checks and other clearances.
• Valid driver's license and reliable transportation preferred.
• Ability to complete required ODP and agency training.
• Previous experience supporting individuals with intellectual disabilities or autism is preferred but not required.

Responsabilités

• Deliver services per ISP and agency policies.
• Assist with ADLs including bathing, dressing, grooming, toileting, mobility, eating, and hygiene.
• Assist with IADLs including meal preparation, housekeeping, laundry, shopping, budgeting, transportation, and meds reminders as authorized.
• Promote independence, community participation, and informed choice.
• Ensure the health, safety, dignity, privacy, and rights of individuals receiving services.
• Observe and report changes in health, behavior, or functional status.
• Complete accurate documentation and EVV requirements.
• Report incidents per ODP requirements.
• Maintain confidentiality and comply with HIPAA and policies.
• Attend required meetings, supervision, and training.
• Perform other duties as assigned.
